All of these must be completed online via the link WARRICK-IN FINAL FORMS below.This is new in 2022 and will require setting up a new account. These must be completed every year.

IHSAA waiver form found below needs to be completed for any USA swimmer choosing to swim a USA meet DURING high school official season. The form must be completed and returned to coach at least 1 week prior to the event. Only 2 waivers are allowed per season.
If you plan to take your athlete home after an away meet, you must complete waiver below & return to coach prior to leaving for meet.
